About the Role

Senior Care Navigator Group is looking for a compassionate and reliable Registered Nurse (RN) to join our home health care team on a per diem basis. The RN will provide skilled nursing services to patients in their homes, ensuring safe, high-quality care that aligns with physician orders and state regulations.

Responsibilities

Conduct initial and follow-up assessments to evaluate patient needs.

Develop, implement, and update individualized care plans.

Administer medications and treatments as prescribed.

Educate patients and family members on care, medication, and health management.

Collaborate with physicians, caregivers, and interdisciplinary team members.

Document care and progress notes accurately and timely in the EMR system.

Supervise HHAs/CNAs and ensure adherence to care plans and regulations.

Qualifications

Required Certifications (Florida):

Active Florida Registered Nurse (RN) License (unencumbered)

Current CPR Certification (AHA or Red Cross)

Level II Background Screening (AHCA)

TB Test or Chest X-Ray (within past 12 months)

Physical Examination (within past 6 months)

Valid drivers license, auto insurance, and reliable transportation

Preferred

At least 1 year of experience in home health care or clinical setting

Familiarity with OASIS documentation preferred

Excellent clinical judgment and communication skills
