Full-time Community Health Worker (SDOH)

Waikiki Health is recruiting for a full-time Social Determinants of Health SDOH) Community Health Worker. ​The SDOH Community Health Worker is responsible for improving outcomes for high-risk patients, who utilize the ED/Hospital regularly and/or that have been identified using our Social Determinants of Health (SDOH) screening tool, PRAPARE. This position will help achieve better outcomes by working with an interdisciplinary team. The Community Health Worker (CHW) will assist in providing services during clinic hours, participating in huddles, patient intakes, education, and counseling, within clinic protocols. ​



Duties include but are not limited to:

- The SDOH CHW is a patient advocate and facilitator of the Patient Centered Medical Home (PCMH) and its target population of high-risk SDOH patients and high ER/Hospital utilizers through the provision of health education, follow-up monitoring, and improvement of access to care.

- Ensures continuity of care through managing the health and social needs of the patient population.

- Creates quarterly reports with cohort data and provides supplemental information to support the narrative

- Acts as a patient advocate in order providing care which is in the best interest of the patient’s health at all times.

- Other duties as assigned

Qualifications:

- Successful completion of a Community Health Worker program or BA in Public Health is preferable.

- Three years of experience in an outpatient setting preferred.

- Transportation and valid driver’s license.

- Familiarity with common software programs, (i.e. Windows-based programs) an ability to quickly learn new systems.

- Ability to communicate effectively with patients, community members, and staff members.

- Proficient in computer skills: typing, navigating through the internet and emails.

- OSHA training.

- BCLS biannually.
