About the Job
Primary Function: Provide clinical case management for clients to ensure that continuing care plans are effectively coordinated and in place upon discharge.

Essential Functions:

Meet with clients throughout their stay to collaboratively determine continuing care needs, and consult with family members and referents as appropriate.
Attend weekly staffing meetings and participate as an active member of the treatment team.
Assist with the development and implementation of a treatment plan that includes continuing care recommendations.
Facilitate communication between clients and continuing care resources to ensure smooth transition to the next level of care.
Participate in conflict resolutions and crisis situations with clients.
Facilitate transfer and discharge processes.
Complete all necessary documentation and record keeping.
Adhere to confidentiality laws including 42CFR Part 2, HIPAA and HITECH as well as all Harmony policies and procedures.
Requirements:

Bachelor’s degree in Psychology, Social Work, or related field, and/or CAC II certification.
Must have valid Colorado driver’s license and acceptable motor vehicle record if driving Harmony vehicle or driving for Harmony business.
If recovering from a substance use disorder, six months of continuous sobriety is required.
Hold a valid First Aid and CPR certification card (training provided).
Computer skills adequate for required tasks. Must be proficient in Microsoft Word and willingness to be trained in other Microsoft Office Programs.
Previous work experience in mental health or substance use disorders.
